We ask that you please bring your driver's license or photo ID, insurance card(s) and any copay that may be due to your first appointment. It is beneficial if you bring your complete medication list or the bottles of your current prescription and over-the-counter medications. This ensures that we can see you promptly and have an accurate record of information for you. If you have medical records being transferred to our office, please ensure that the appropriate forms are completed and returned so we can request them in a timely manner.
